Position Overview
We are looking for a dedicated Physical Therapist to become part of a collaborative outpatient rehabilitation team. Key Responsibilities
Patient Assessment & Treatment
• Conduct comprehensive evaluations to assess patients' physical limitations, mobility, and functional abilities.
• Develop individualized treatment plans based on clinical findings and patient-specific goals.
• Deliver evidence-based physical therapy interventions to promote recovery, reduce pain, and improve overall function.
• Reassess patient progress regularly and adjust treatment plans to maximize outcomes.
Patient Education
• Educate patients and family members on prescribed exercises, recovery expectations, injury prevention, and techniques to maintain long-term physical health.
• Encourage active participation in rehabilitation by providing guidance on home exercise programs and self-management strategies.
Documentation & Clinical Records
• Maintain complete, accurate, and timely documentation of evaluations, treatment sessions, progress notes, and discharge summaries.
Care Coordination
• Collaborate with physicians, referring providers, and other members of the healthcare team to coordinate comprehensive patient care.
• Communicate patient progress and recommendations to appropriate healthcare professionals to support continuity of care.
Qualifications
Education
• Doctor of Physical Therapy (DPT) degree from an accredited program. Licensure
• Current and active Florida Physical Therapist license.
