- Lead and manage the facility’s social services program
- Conduct psychosocial assessments, care planning, and documentation
- Serve as a primary advocate for resident rights, dignity, and emotional well-being
- Guide residents and families through admissions, care transitions, and discharge planning
- Coordinate and lead care conferences and behavioral interventions
- Collaborate with the interdisciplinary team (nursing, therapy, leadership)
- Assist families with resources, financial processes, and long-term planning
- Ensure compliance with federal and state regulations for social services in SNF
- Provide staff education on communication, behaviors, and resident-centered care
- Bachelor of Social Work (BSW) required
- Skilled Nursing Facility (SNF) / Long-Term Care (LTC) experience required
- Psychosocial assessments & care planning
- Discharge planning & care transitions
- Regulatory compliance in long-term care
- Excellent communication, documentation, and interpersonal skills
- Ability to build trust with residents, families, and team members
- Strong organization and time management
- You are compassionate, calm, and solution-focused
- You are confident advocating for residents and families
- You enjoy leading programs and influencing outcomes
- You thrive in a team-oriented, fast-paced healthcare environment
