Health Services Management (HSM), a not-for-profit, multi-state operator of skilled nursing facilities, is seeking a highly experienced MDS Coordinator to support our Texas communities within a 100-mile radius of the Houston area.

This is a regional, hands-on clinical leadership role, not remote, ideal for an MDS expert who thrives in the field, partnering directly with facility teams to drive accuracy, compliance, and reimbursement excellence.

What You’ll Do

• Support & Strengthen Facility MDS Teams
• Serve as a clinical expert and mentor to MDS Coordinators across assigned facilities
• Provide hands-on guidance with MDS accuracy, PDPM scoring, ICD-10 coding, and regulatory updates
• Assist with complex clinical documentation and case review
• Audit, Monitor & Ensure Compliance
• Conduct routine and focused MDS audits for accuracy and timeliness
• Identify missed reimbursement opportunities and documentation gaps
• Partner with facility leaders on corrective action plans and education
• Drive Reimbursement & Quality Outcomes
• Ensure coding reflects true clinical acuity to support PDPM reimbursement optimization
• Analyze and interpret:
• Case Mix Index (CMI)
• QIPP
• Quality Measures (QMs)
• PDPM performance indicators
  
  

Provide oversight and education on:

• CMS Quality Measures (QMs)
• Quality Reporting Program (QRP)
• Value-Based Purchasing (VBP)
• Texas QIPP initiatives
• Training & Clinical Education
• Lead onboarding and ongoing training for MDS staff
• Deliver on-site and group education sessions
• Keep teams current on CMS updates, MDS changes, and documentation standards
• Regional Oversight & Reporting
• Track and trend performance metrics across buildings
• Serve as a liaison between facility teams and corporate leadership
• Support consistency, accountability, and continuous improvement across the region
  
  

What You’ll Need To Be Successful

• Required: Proven MDS experience in a skilled nursing (SNF/LTC) setting
• Strongly Preferred: Active Registered Nurse (RN) license in good standing
• 3–5 years of MDS experience, including multi-site or leadership exposure
• RAC-CT certification (or ability to obtain within 6 months)
  
  

Deep knowledge of:

• MDS 3.0
• RAI
• PDPM & CMI
• CMS reimbursement models
• Experience with PointClickCare, SimpleLTC, and/or Net Health
• Strong auditing, analytical, and coaching skills
• Must reside near the Houston area and be able to travel within a 100-mile radius
• Ability and willingness to support facilities on-site, regularly
  
  

Important Role Details

This is a regional Texas-based position (Houston area)

Not a remote role, consistent on-site facility presence is required

Travel is essential to support multiple buildings within the assigned region
