Essential Duties And Responsibilities
Medication Administration
- Administer medications as ordered by the physician and delegated by the licensed nurse.
- Prepare, distribute, and document medications according to facility policy and state regulations.
- Observe residents for adverse reactions, side effects, or changes in condition and promptly report findings to the licensed nurse.
- Maintain accurate medication administration records (MARs) and treatment administration records (TARs).
- Ensure medications are stored, handled, and disposed of according to facility policies and regulatory requirements.
- Conduct medication cart audits and maintain medication cart cleanliness and organization.
- Provide routine personal care assistance as needed within scope of practice.
- Assist residents with activities of daily living (ADLs) when required.
- Monitor and report changes in residents' physical, emotional, or mental status.
- Promote resident rights, dignity, privacy, and independence.
- Accurately document medication administration, treatments, observations, and resident responses.
- Report medication errors, incidents, or variances immediately to the supervising nurse.
- Complete required reports and documentation in a timely manner.
- Follow infection prevention and control procedures, including hand hygiene and use of personal protective equipment (PPE).
- Maintain compliance with CMS, state, facility, and professional standards.
- Participate in quality assurance and performance improvement activities.
- Attend required in-services, meetings, and educational programs.
- Communicate effectively with residents, families, physicians, nurses, and other interdisciplinary team members.
- Assist in orienting and mentoring new staff as assigned.
- Maintain a positive and professional working relationship with coworkers and residents.
Education
- High school diploma or equivalent preferred.
- Current Certified Medication Aide certification in the State of Texas.
- Current Certified Nurse Aide (CNA) certification in good standing preferred.
- Current CPR certification preferred.
- Experience in long-term care, skilled nursing, or rehabilitation preferred.
- Knowledge of medication administration practices and documentation systems preferred.
- Knowledge of basic pharmacology and medication administration principles.
- Ability to accurately read, understand, and follow physician orders.
- Strong observation and communication skills.
- Ability to maintain confidentiality and professionalism.
- Ability to work independently and as part of a healthcare team.
- Basic computer and electronic health record (EHR) skills.
- Ability to lift, transfer, and reposition residents as needed.
- Ability to stand, walk, bend, stoop, and reach for extended periods.
- Ability to push medication carts and transport supplies.
- Ability to perform duties requiring visual acuity and manual dexterity.
- Exposure to infectious diseases, bodily fluids, and potentially hazardous materials.
- May be required to work weekends, holidays, overtime, and emergency staffing situations.
- Subject to frequent interruptions and stressful situations requiring independent judgment.
