What are the responsibilities and job description for the Front Office Patient Care Coordinator position at Health Service Alliance?
ESSENTIAL DUTIES AND RESPONSIBILTIES:
· Organizes and maintains a filing system for patient charts, including filing and pulling charts. Sorts and verifies that all clinical documents, appropriate diagnosis codes, and other referral-related documents are completed or filed in accordance with HSA policies. Keeps accurate log and records all patient referrals in appropriate data base as assigned.
· Answers telephone and either responds to inquiry, directs caller to appropriate personnel or takes a written message for the appropriate personnel. Assists with patient phone messages/tasks and documents in EMR. Conducts callbacks per provider instruction in an efficient and timely manner, while maintaining proper telephone etiquette.
· Schedules appointments and enters appointment date and time into computerized scheduler.
· Sends out reminder calls to all patients.
· Greets and directs patients, salespeople, vendors, reps and new providers.
· Registers patients by verifying that patient’s record is up to date and accurate. Makes appropriate changes in computer system and in patient’s chart or account.
· Prepares all billings in computerized system for processing by Business Office.
· Collects payment from patients and reconciles daily cash reports.
· Process all client requests in an appropriate and timely manner.
· Skill to appropriately address and interact with clients that seek services with the understanding that clients will, at times, express extremes of emotions due to the nature of their visit(s).
· Communicates with appropriate health plans, providers, care team members and available community resources to promote quality, timely, and effective outcomes. Contact insurance companies to ensure prior approval requirements are met.
· Responds to incoming telephone calls regarding pending referrals, including timely response to voice mail messages. Coordinates referrals for patients through insurance and other physician offices as needed.
· Reviews and investigates, and relays information to requesting provider of claim denials, specialty provider calls for coordination as needed.
· Assembles information concerning patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical documentation (clinical notes, radiology, labs) to specialist.
· Attends and actively participates in mandatory clinical/business department meetings/in-services, staff meetings and assigned committees.
· Communicates, orally and in writing, appropriately and clearly to physicians, staff, patients and their families, the Leadership Team, and outside entities as appropriate. Maintains records pertinent to personnel and operations of HSA.
· Adheres to, complies with, and demonstrates support for the mission, policies and procedures of HSA.
· Assures that all HIPAA regulations are met; participates in education as needed. Maintains appropriate patient and provider confidentiality.
· Attends meetings as assigned. Actively participates in organization-wide activities and projects.
· Interacts and collaborates respectfully with patients and their families. Sets example for all staff when working with patients and their families to identify and address psychosocial, cultural, ethnic and religious/spiritual needs.
· Participates and adheres to employee safety program for HSA staff and attends training as required.
· Participates in role specific training.
· Is responsible to receive appropriate immunizations and other health services as deemed necessary.
· All other duties as assigned.
QUALIFICATIONS:
Education/Experience
High school Diploma or GED required. At least one year experience in a healthcare setting performing related job duties.
