Prototypes is one of the nation’s leading behavioral healthcare providers and is a “prototype” for such
organizations. Prototypes Women's Center in Pomona is a 180-bed residential facility tailored specifically
to women, including those who wish to involve their children in their journey to recovery. Our program
aims to assist women in maintaining their progress in overcoming co-occurring disorders through a wide
range of supportive services. Children can foster a secure attachment with their mothers in a secure
setting, while also accessing mental health support and educational assistance. Our interdisciplinary team
of professionals provides comprehensive treatment for substance use disorders and mental health,
parenting, educational, and life skills courses. Treatment can range from 60 days to 6 months.
The Intake Coordinator I is responsible for providing in-person initial assessment of candidates to
determine medical necessity, completion of program admission forms, ASAM assessment, and urgent
referrals regarding medical and mental health needs. The Intake Coordinator I works in conjunction with
the intake team to provide a warm hand off to the client’s treatment team, assuring new clients are
stabilized, and urgent needs have been addressed. The Intake Coordinator I will work closely with
Counselors, Case Managers, Therapists, and other team members to coordinate and monitor client issues.
In addition, Intake Coordinator I provides individual and group counseling, psychoeducation, relapse
prevention, harm reduction, crisis intervention, and case management.

KEY RESPONSIBILITIES
Direct Service
- Conduct initial assessment and intake evaluation for the formulation of treatment plan.
- Consult regularly with other staff to ensure clients meet criteria for safe admission to an
- Provide case management services (e.g. housing, employment, finance management, court
- Identify and implement relapse prevention or harm reduction interventions when indicated.
- Provide group, family, and individual counseling.
- Facilitate group sessions focused on substance use recovery, trauma recovery, co-occurring
- Routinely review treatment plans.
- Maintain compliance with the due dates for all clinical documentation.
- Maintain confidential client records and prepare client progress reports and other required
- Accurately enter data in various electronic systems for all clients in accordance with guidelines
Administration and Other Duties
- Establish community relations and linkages with other service providers and community services.
- Actively participate in clinical supervision, case conferences, and treatment team reviews of client
- Attend meetings and actively participate.
- Complete assigned training on time.
- Meet service delivery goals.
- Read and respond to communications in a timely manner.
- Maintain full compliance with credential requirements.
QUALIFICATIONS
Education, Certification, or Licensure
- High school diploma or equivalent required. Bachelor’s degree in Health and Human Services,
- Registration or Certification as a Substance Use Disorder Counselor from an accredited California
- Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
- At least one year of experience working in the substance abuse treatment field.
- Experience in culturally diverse substance abuse populations with co-occurring disorders or with
- Experience conducting assessments and providing appropriate diagnoses.
- Experience with mental health Medi-Cal procedures.
- Familiarity and/or experience with Drug Medi-Cal and ASAM Criteria preferred.
