As we expand our services, we are seeking an experienced Lead Care Manager to support our Enhanced Care Management program.
Job Summary:
We are seeking a dedicated and compassionate Lead Care Manager (LCM) to join our team. As a LCM, you will play a pivotal role in our Enhanced Care Management (ECM) program under California Advancing and Innovating Medi-Cal (CalAIM). This position is essential to improving the health and social outcomes of our ECM population by providing comprehensive care management and coordination services. The ideal candidate will have a strong background in care management, social work, and a passion for serving vulnerable communities.
Responsibilities:
- Conduct outreach and engagement activities to identify and establish rapport with ECM-eligible members.
- Complete comprehensive health and psychosocial assessments to develop individualized, member-centered care plans.
- Coordinate care by collaborating with members' care teams, families, and community providers; this includes scheduling appointments, arranging transportation, and following up on referrals.
- Implement and monitor member care plans, providing education, teaching self-management skills, and addressing barriers to care.
- Document all care management activities and maintain detailed progress notes.
- Collect and review member health data continuously to identify gaps in care and opportunities for improvement.
- Collaborate with clinical consultants and medical providers to adjust care plans as needed.
- Offer support and guidance by connecting members to appropriate social services, community resources, and external counseling providers, while assisting with care coordination and crisis support as needed.
Requirements:
- A minimum of 2 years’ experience in care management, case management, or a related field (preferred).
- Strong knowledge of social determinants of health and community resources.
- Excellent communication, problem-solving, and care coordination skills.
- Passion for connecting underserved populations with resources to improve their health.
- Bachelor’s degree in social work, healthcare, or a related field (preferred).
- Familiarity with CalAIM and experience serving Medicaid, dually eligible, or uninsured
- populations is a plus.
- Bilingual in English and Spanish is required.
- Reliable transportation and willingness to travel to meet members in their homes, community settings, and provider locations as needed.
