What are the responsibilities and job description for the Diabetes Educator- Kentucky Licensed position at HEALTH POINT FAMILY CARE INC?
HealthPoint is currently hiring for a position that will enhance the lives of their patients. This new position will be working full time in all Northern Kentucky locations: Covington, Newport and Florence locations.
$3,000 sign on bonus paid after 90 days

40 hours per week, with 3, 4 or 5 days per week in office options.

This position will work in 3 offices in NKY, Newport, Covington and Florence

Starting wage $40 per hour with 2 or more years experience, $35 per hour if less than 2 years experience

Provides one-on-one diabetes and nutrition education to HealthPoint diabetic patients in a clinic setting

Educates and designs meal plans for patients that are are affordable and accessible with easy to understand recipes

Monitors patient progress, adjusting meal plans and education as needed, and providing ongoing support and encouragement through in person follow up visits

Uses a variety of teaching strategies, approaches, and methods that incorporate theories and concepts that are sensitive to the learning preferences of the individual, reading level, readiness for change, empowerment and motivational interviewing, and local cultural values.

Interprets personal health data, develops an individualized care plan based on the patient’s assessed needs, goals and cultural beliefs and promotes successful self-management.

Participates in organizational ADCES Accreditation Process
