Responsibilities

General Description The Health Equity Program Specialist supports implementation of HPSM’s Health Equity Strategy and CLAS Program activities including the design of interventions aimed at addressing health inequities and reducing health disparities among HPSM’s member population. HPSM’s Health Equity unit’s focus areas include Addressing Health Disparities, promoting culturally inclusive care, providing linguistically appropriate services and fostering community partnerships for input and collaboration. Duties & Responsibilities Essential Functions:

  • Actively contributes to the ongoing development and participates in the execution of HPSM’s Health Equity Strategy and associated initiatives.
  • Contributes to the maintenance of health equity and population health program documents, maintains health equity workplans and implementation progress tracking documents.
  • Conduct ongoing disparities analysis and assessments of specific subpopulations to identify disparities among HPSM’s membership and prioritize areas for intervention and inform the planning process.
  • Foster collaborative partnerships with cross-departmental teams, community partners, provider network and HPSM members to collect input in the intervention design process.
  • Design targeted member outreach strategies to disparate or at-risk member subgroups to connect members to programs, services or close gaps in care.
  • Lead language assistance services (LAS) functions related to accessing interpreter services, vendor relationship management, monitoring utilization and investigating member complaints against interpreter services vendor as needed.
  • Serves as Co-facilitator of Health Equity focused topic task force working groups including tracking of task force activities and progress
  • Supports implementation of Language Assistance Services (LAS) requirements from regulatory agencies such as DHCS, DMHC, NCQA.
  • Participate in various provider and HPSM staff education opportunities focused on culturally inclusive care by preparing and presenting communications (e.q. awareness months, best practices resources, etc.)
  • Create process documentation and workflows for health equity activities and health disparities interventions.
  • Participate cross-functionally in planning to support HPSM initiatives with a focus on improving health outcomes, health equity, accessibility, and reducing health disparities.
  • Educate and work with external partners as needed to identify innovation opportunities and improve processes and outcomes. Secondary Functions:
  • Perform other duties as assigned Qualifications - The following represents the typical way to achieve the necessary skills, knowledge and ability to qualify for this position. Education and Experience equivalent to:
  • Bachelor’s Degree or equivalent work experience in Public Health, Health Education/Promotion, Social Work, Sociology, or a related field. Master’s in public health or administration (MPH/MPA) strongly preferred.
  • Three (3) years of experience in public health or healthcare setting, working with diverse populations, project management or program development and evaluation. Experience with Medi-Cal and Medicare programs/members preferred. Knowledge of:
  • Population Health, Health equity concepts and terminology.
  • Health Disparities concepts and analysis methods
  • Organizational behavior skills with a focus on driving for results.
  • Project management and intervention planning Skills:
  • Commitment to social justice and passion for advocacy work.
  • Experience engaging with diverse communities or vulnerable populations
  • Experience planning health equity/health disparities intervention.
  • Communicate effectively, both verbally and in writing.
  • Personal computers and proficiency in Microsoft Office Suite applications, including Outlook, Word, Excel, Access and PowerPoint.
  • Relationship building and interpersonal skills.
  • Think critically and make informed decisions. Ability to:
  • Communicate effectively and produce well-organized and informative assessments, reports and presentations.
  • Work cooperatively with others.
  • Work as part of a team and support team decisions.
  • Adapt to changes in requirements/priorities for daily and specialized tasks.
