Position Overview

The Behavioral Health Wraparound Coordinator supports children and youth with serious behavioral health challenges by coordinating a structured, family-driven planning process that promotes stability, resilience, and success in the home and community.

 

This role serves as the primary point of contact for youth and families, partnering with service providers and natural supports to develop and implement individualized, strength-based care plans that address immediate needs and long-term goals.

 

The coordinator facilitates collaboration across systems—including schools, healthcare providers, and community organizations—to ensure coordinated, effective service delivery and improved outcomes for youth and families.

Key Accountabilities:


Implement the High Fidelity Wraparound (HFW) process in alignment with established guidelines and best practices

  • Facilitate the High Fidelity Wraparound process for youth and families with complex behavioral health needs.
  • Conduct ongoing assessments of strengths, needs, and progress.
  • Maintain regular communication with all team members to ensure coordinated service delivery.
  • Track progress, referrals, and outcomes in accordance with program requirements.
  • Document all interactions, services, and updates in appropriate systems in a timely manner.
  • Contribute to a collaborative, team-based work environment.


Engage youth and families to identify strengths, needs, and goals

  • Build trusting, collaborative relationships with youth, families, and team members.
  • Seek consultation and coaching when needed to support effective service delivery.

 

Develop and facilitate individualized care plans that promote stability and long-term success

  • Engage families in identifying strengths, setting realistic short-term goals, and developing a long-term vision.
  • Co-create individualized, strength-based care plans that address priority needs
  • Ensure plans are family-driven and culturally responsive.
  • Support families in building confidence and skills to manage services and advocate for their needs.

 

Coordinate multidisciplinary teams of providers and natural supports

  • Recruit, prepare, and coordinate multidisciplinary teams, including service providers and natural supports.
  • Facilitate regular team meetings that promote collaboration, accountability, and progress.
  • Serve as a liaison between families and providers to reduce barriers and improve communication.
  • Coordinate services across systems such as schools, healthcare providers, and community organizations.
  • Participate in regular supervision, team meetings, and professional development opportunities.


Monitor and evaluate care plan effectiveness and adjust strategies as needed

  • Monitor implementation of care plans and adjust strategies as family needs evolve.
  • Track outcomes and celebrate progress to strengthen engagement and empowerment.
  • Assess and support development of self-efficacy in youth and families.
  • Share progress and outcomes with supervisors and team members as required.

 

General Requirements & Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Counseling, or a related human services field.
  • Experience working with children, adolescents, and families with behavioral health challenges or complex needs.
  • Strong facilitation, communication, and collaboration skills.
  • Ability to coordinate services across multiple systems.
  • Commitment to family-centered and strength-based approaches.
  • Strong organizational, problem-solving, and follow-through skills.
  • Ability to engage diverse populations with empathy and cultural competence.
  • Effective written and verbal communication skills.
  • Willingness and ability to complete HFW Facilitator training.
  • Understanding of and experience with child-serving systems (education, behavioral health, child welfare, juvenile justice, healthcare, etc.).
  • Genuine respect for families of children/youth with complex behavioral health needs and commitment to empowerment.
  • Extensive knowledge of community resources.
  • Access to reliable transportation and a valid driver’s license.
  • Availability during regular work hours, with some evenings and weekends.
  • Must provide proof of a valid driver’s license and adequate insurance coverage totaling at least $300,000 per occurrence.

Working Conditions:

  • Up to 10% time traveling and flexible work schedule with some work remotely.  
  • Work location for administrative activity is in an accessible office environment.  
  • Daily activity is 80% sitting or standing with extended periods of typing at a keyboard, 20% walking with occasional stooping, bending, reaching, twisting. 
  • Office equipment would include phone, computer, printer, copier on a daily basis. 
  • Independent travel throughout the region, including during inclement weather. 

 

Compensation:

This is a salaried, non-exempt position, compensated on a salary basis and eligible for overtime pay for all hours worked over 40 in a workweek, in accordance with applicable state and federal laws.

 

$56,650-$59,000 per year.
