Licensed Practical Nurse (LPN) - PedsBryan Community Health CenterFull-time : Monday-Friday 8 : 00am - 4 : 30pmNo Weekends, No HolidaysPOSITION PURPOSE : The Licensed Practical Nurse (LPN) assists the Medical Provider or Registered Nurse (RN) in the care and treatment of Health Partners of Western Ohio's patients. The nurse performs nursing services utilizing the nursing process. The LPN shall perform a variety of complex nursing tasks and related duties as required in a patient centered medical home setting.QUALIFICATIONSEDUCATION / CERTIFICATION : Graduate of a school of nursing accredited by National League of Nursing.Current license to practice as a Licensed Practical Nurse in the State of Ohio.REQUIRED KNOWLEDGE : Successful completion of in-house training.EXPERIENCE REQUIRED : SKILLS / ABILITIES :
- CPR - BLS (basic life support) required.
- One to three years full-time experience in nursing practice is preferred.
- Ability to work as a team member.
- Computer experience desirable.
- Ability to work with a culturally diverse group of people.

- Collects patient data through physical examination techniques, interview, and observation.
- Documents all nursing care provided according to policy.
- Formulates plan of care for clients under direction from providers.
- Obtains referrals from other physicians and specialists.
- Administers medications as prescribed by the provider.
- Follows approved protocols / standing orders for client care.
- Follow universal precautions.
- Performs treatments, procedures as prescribed by the provider.
- Performs technical skills.
- Responds to client / family / caregiver needs.
- Responds to crisis situations according to policy.
- Utilizes client education for emotional and physical health maintenance.

- Performs other duties as assigned.WORK ENVIRONMENTWork is primarily sedentary, demanding sitting, walking, lifting, and bending. Those physical movements and the degree of mobility, manual dexterity, and hand-eye coordination commonly associated with duties in an office setting will be performed repetitively.
