Manage treatment plans in an SNF setting to promote positive patient outcomes for diverse diagnoses. Provide skilled physical therapy services during days, typically 8-10 hour shifts, 4-5 days a week. Responsible for assessing patient needs, implementing therapy interventions, and documenting progress. Collaborate with healthcare teams to ensure quality care and compliance with regulations.
Qualifications & Skills
Active state license, current BLS (AHA preferred), and supervisory professional references. Must complete drug and background screens. Experience in SNF preferred. Strong communication, assessment, and patient management skills are essential.
