- Providing high-quality nursing care to adult patients with complex needs, including trach/vent support and use of Hoyer lifts.
- Ensuring clinical outcomes through compassionate and empathetic homecare.
- Utilizing electronic documentation via tablets in patients' homes.
- Collaborating with a multidisciplinary team and supporting patients and families.
- Active, unencumbered LPN/LVN license in the practice state.
- Current CPR certification with hands-on component.
- TB skin test within the last 12 months.
- Reliable transportation.
- Preferred but not required: six months of hands-on nursing experience.
- Strong clinical skills, compassion, and empathy.
- Ability to work flexible shifts, including weekends.
- Excellent communication and teamwork abilities.
