- Providing direct patient care for medical/surgical needs, assessing and managing patient needs under RN oversight
- Performing routine nursing procedures, coordinating care with the healthcare team
- Developing and revising patient care plans in collaboration with RNs and physicians
- Educating patients and caregivers to promote self-care and support
- Ensuring accurate documentation and communication within the care team
- Participating in staff meetings, in-services, and ongoing education
- Serving as a preceptor for new LPN staff and participating in call rotation and weekend visits
- High School Diploma/GED and valid LPN license
- Current BLS certification
- Minimum of 1 year medical/surgical nursing experience in an acute care setting (preferred)
- Strong assessment, clinical, organizational, and communication skills
- Proficiency with Windows-based systems and home health software
