Licensed Practical Nurse / Licensed Vocational Nurse - Home Health - $28-32 per hour

Role: Licensed Vocational / Practical Nurse (LPN/LVN) in Home Health, responsible for providing high-quality patient care in a home setting.

Key Responsibilities

• Verify and implement the Plan of Care based on physician orders, RN delegation, and assessment data.
• Deliver patient-centered care, incorporating care goals and documenting progress.
• Communicate significant findings and changes in patient condition to the healthcare team.
• Ensure patient safety and assist with self-care education.
• Collaborate with RNs and physicians to revise care plans as needed.
• Document medication administration and updates accurately.
• Assist in preventing hospitalizations and provide supportive communication to patients and families.
  
  

Qualifications & Skills

• Graduate of an accredited nursing program with an active state license.
• Minimum of one year of clinical care experience; experience in hospice or home health preferred.
• Ability to lift, support, and transfer patients up to 50 pounds.
• Valid driver’s license, insurance, and reliable transportation.
