- Providing and documenting skilled nursing care in accordance with care plans and physician orders.
- Assessing patient conditions, reporting changes, and implementing appropriate interventions.
- Educating patients and families on disease management, self-care, and prevention strategies.
- Maintaining compliance with policies, regulations, and safety standards, including infection control.
- Participating in ongoing education and skill development.
- Active, unencumbered LPN/LVN license in the practice state.
- Current CPR certification and TB test (within last 12 months).
- Preferably six months of recent clinical experience; pediatrics experience is a plus.
- Reliable transportation and physical ability to lift up to 50 pounds and perform patient repositioning.
- Attention to detail, effective communication, and time management.
- Ability to problem-solve and handle emergency situations.
- Adaptability to varied environments and conditions.
- Providing patient care in home settings, including feeding tube management and medication administration.
- Monitoring patient progress, documenting care activities, and updating care plans.
- Ensuring safety and infection control measures are followed.
- Educating patients and families to promote health and safety.
