TITLE: Patient Care Coordinator/Abortion Navigator/Medical Assistant

PROGRAM: Health Imperatives Sexual & Reproductive Health Services

HOURS: Fulltime, Monday through Friday including some evenings

SALARY: $32/HR

BENEFITS: Medical, dental, holiday, paid earned leave, 403b, etc.

DUTIES: Patient Care Coordinator/Medical Assistant will work within the Sexual and Reproductive Health Services

team and in partnership with patients to help address barriers to care and achieve better health outcomes

by:

  • Serving as the primary point of contact for patients seeking abortion services
  • Conducing patient pre-visit intakes, review consent processes, appointment expectations, and logistical needs
  • Assisting patients with insurance navigation, prior authorizations, Abortion Fund navigation, and billing
  • Triaging patient concerns pre and post medication and procedural abortion services
  • Tracking patient referrals, support patient navigation to outside medical care, provide follow-up post referral, and obtain medical records for clinician review
  • Performing routine lab functions and testing including phlebotomy, urine pregnancy testing, HIV and Hepatitis testing and preparing specimens for transport
  • Assisting in procedural abortion clinics to provide clinician and patient support. taking vital signs, providing aftercare, and patient support and information
  • Assisting with GYN procedures including LEEP, colposcopy, and ultrasound
  • Rooming patients and obtaining and recording client’s personal medical history including gynecological, obstetrical and sexual history
  • Developing and maintaining up-to-date referral list of surgical abortion, medical, and social service providers
  • Completing accurate and timely documentation in the EMR, logs, and report system
  • Acting as a liaison within HI programs and with community providers
  • Providing patient-centered sexual and reproductive health education and counseling on topics including pregnancy options, abortion services, contraception, sexually transmitted infections, HIV, Hepatitis, PrEP, nPEP, gende affirming hormone therapy, overdose education and Narcan distribution
  • Other duties as assigned

REQUIRED QUALIFICATIONS: Associates degree and at least 2 years of experience providing patient health education, care coordination, and/or or case management; strong client-centered skills; demonstrated ability to communicate unbiased reproductive health knowledge to adolescent and adult patients in an effective and culturally competent, non-judgmental manner; experience working with diverse populations; excellent written and oral communication skills in English; work independently as well as collaboratively within a team environment; computer literacy essential.

PREFERRED QUALIFICATIONS: Bilingual Portuguese speaking; bachelor’s degree in health or human services or minimum of five years’ experience; case management or health navigation experience particularly regarding sexual and reproductive health and abortion related support services; demonstrated experience providing sexual and reproductive health education.
