Nurse Navigator - Oncology Clinic

Role Overview

The Nurse Navigator (NN) is a licensed registered nurse with disease-specific expertise, providing individualized support to patients, families, and caregivers to overcome barriers to care. They facilitate informed decision-making and ensure timely access to quality health and psychosocial services throughout the cancer continuum. The role involves patient and provider interactions via in-person, telephonic, and electronic communication, focusing on promoting care access and adherence to treatment plans.

Key Responsibilities

• Serve as a subject matter expert in navigation, improving care timeliness and access.
• Collaborate with program leaders to evaluate community needs and improve patient navigation programs.
• Deliver patient-centered care tailored to diverse populations, practicing evidence-based nursing processes.
• Coordinate care across specialty clinics and multidisciplinary teams, bridging service gaps.
• Mentor and support professional development of nursing staff, participating in performance improvement and professional activities.
• Ensure compliance with policies, standards, and regulatory requirements.
  
  

Qualifications & Skills

• Associates or Bachelor's nursing degree (preferred).
• Valid RN license (single-state or multi-state compact).
• Certifications: BLS required upon hire.
• Minimum of 3 years of nursing experience, with 2 years in disease-specific or outpatient care preferred.
• Strong critical thinking, time management, and ability to work in a fast-paced environment.
