Role Overview: The Licensed Practical Nurse/Licensed Vocational Nurse provides essential medical and psychiatric care to individuals with disabilities, brain injuries, or behavioral challenges.

Key Responsibilities:

• Monitor health conditions and ensure quality services
• Administer medications, document, and oversee staff medication administration
• Implement prescribed medical treatments and personal care
• Conduct assessments and develop healthcare plans, responding to condition changes
• Train staff on infection control, medication procedures, and safety protocols
• Serve as liaison with medical providers and coordinate external services
• Provide health education including self-medication, STD prevention, and smoking cessation
  
  

Qualifications & Skills:

• Associate’s Degree in Nursing or related field, or state certification
• At least one year of relevant nursing experience
• Current state LPN or LVN license
• Valid driver’s license, vehicle registration, and insurance
• Certifications such as CPR/FA, HIV/AIDS, BBP, and ACT as required
• Strong communication skills with staff, individuals, and families
