Provide high-quality nursing care in community-based settings, supporting individuals with developmental disabilities, brain injuries, and behavioral challenges.
Key Responsibilities Include
- Providing basic medical treatment and personal care services
- Conducting health assessments and collaborating on care plans
- Monitoring, administering, and documenting medications
- Ensuring compliance with physician orders and follow-up on medical appointments
- Performing quality assurance on healthcare records and maintaining confidentiality
- Partnering with external agencies for coordinated services
- Training staff and individuals on medication, safety, and health topics
- Assisting with household activities and transportation as needed
- Participating in on-call emergency coverage
- Associate Degree in Nursing or equivalent LPN training
- Minimum one year of relevant experience
- Current state LPN license and relevant certifications (CPR/FA, HIV/AIDS, BBP, ACT)
- Valid driver’s license, auto insurance, and vehicle if transportation is required
- Strong communication, organization, and compassionate skills
