- Provides patient education on diabetic diets, food choices, eating behaviors, and community resources, tailored to age-specific needs.
- Collaborates with patients, families, and healthcare teams to assess DSMES (Diabetes Self-Management Education and Support) needs and set goals.
- Performs medical nutrition therapy, including screening, assessment, diagnosis, intervention, and monitoring, utilizing the Nutrition Care Process.
- Documents patient encounters, evaluates outcomes, and participates in quality improvement initiatives.
- Ensures compliance with regulatory standards, ethical practices, and organizational policies.
- Shares expertise through presentations, community events, and mentorship of interns and staff.
- Supports professional development through ongoing education, self-assessment, and participation in research and team activities.
- Ability to independently perform clinical functions and adapt education to diverse patient needs.
