The Care Coordinator RN (CC RN) is responsible for assessing patient needs, coordinating care across the continuum, and facilitating smooth transitions from hospital to next care level.
Key Responsibilities
- Assess clinical, psychosocial, and social determinants of health to develop individualized discharge and care plans.
- Collaborate with patients, families, physicians, and interdisciplinary teams to ensure timely care progression and discharge.
- Manage discharge planning, identify barriers, and facilitate post-acute referrals.
- Coordinate communication on patient progress, resolve delays, and ensure documentation accuracy.
- Participate in interdisciplinary rounds and support patient education.
- Associate's or Bachelor's Nursing degree; RN license (single or multi-state compact); BLS certification.
- Minimum of 1 year nursing experience in an acute care setting.
- Strong communication, assessment, organizational, and problem-solving skills.
- Knowledge of healthcare regulations, community resources, and care transition processes.
- Ability to work collaboratively, handle stress, and adapt in a fast-paced environment.
