Behavioral Health Care Coordinator

Integrated Care Extender supports delivery of community-based behavioral health services by assisting clinicians with client engagement and care coordination. The role focuses on working with children, adolescents, adults, and families to strengthen follow-up, continuity of care, and treatment implementation.

• Coordinate outreach and scheduling to connect clients with needed services
• Support assessments and help implement treatment plans as directed by clinicians
• Maintain accurate documentation of services and activities
• Link clients to community resources and support continuity across settings
• Collaborate with a multidisciplinary team to support client-centered, trauma-informed care
• Promote safety, recovery, and improved outcomes in home, office, and community environments
  
  

Requires strong interpersonal communication, organizational skills, and the ability to provide compassionate, trauma-informed support while working closely with clinicians and community partners.
