The APP position is to provide clinical coverage as part of an advanced practitioner team within the Advanced Cellular and Gene Therapy Unit. This position will work across both inpatient and outpatient settings, managing the full continuum of transplant care. This APP position will complete thorough evaluations of pediatric bone marrow transplant patients (from neonates to adolescents and young adults) with increasing independence, while working closely with a multidisciplinary team of physicians, nurses, and advanced practitioners. Recommends broad evaluation based on prioritized focused differential diagnosis. Interprets clinical significance of diagnostic study results. Collaborates care with the supervising physician and inter-professional team. Develops an interdisciplinary management plan for common and typical diagnoses. Continues to develop competencies in core procedures.

Clinical Care – Inpatient & Outpatient

  • Manage daily care of pediatric BMT patients in both inpatient units and outpatient clinics.
  • Conduct comprehensive assessments, develop treatment plans, and adjust therapies in collaboration with the transplant team.
  • Monitor for and manage transplant-related complications, including graft-versus-host disease, infections, organ dysfunction, and treatment toxicities.
  • Provide continuity of care throughout the transplant process, from pre-transplant evaluation through long-term follow-up.
  • Coordinate care transitions between inpatient and outpatient settings to ensure seamless patient experiences.

Patient & Family Education

  • Deliver ongoing education to patients and families regarding transplant processes, medications, symptom management, infection prevention, and long-term expectations.
  • Support families in understanding complex medical information and making informed decisions.
  • Develop and update educational materials and participate in teaching sessions, classes, or support groups.

Procedures

  • Perform advanced procedures such as lumbar punctures, bone marrow aspirates/biopsies, bone marrow harvest, and other skills within scope of practice.
  • Assist with or independently perform diagnostic and therapeutic interventions as needed.
  • Ensure procedural safety, informed consent, and appropriate follow-up care.

Collaboration & Coordination

  • Work closely with physicians, nurses, pharmacists, social workers, and other specialists to deliver integrated transplant care.
  • Participate in multidisciplinary rounds, care conferences, and quality improvement initiatives.
  • Communicate effectively with referring providers and community partners.

Quality, Safety & Professional Development

  • Adhere to institutional protocols, regulatory standards, and evidence-based transplant guidelines.
  • Participate in research, data collection, and quality improvement projects related to BMT outcomes.
  • Maintain advanced practice certification and engage in ongoing professional education in pediatric oncology and transplant medicine.

Medical knowledge

  • Clinical knowledge . With increasing independence, applies an intermediate-level of medical knowledge to common and typical scenarios, incorporating some literature to support care.
  • Diagnostic Evaluation. Recommends broad evaluation based on prioritized focused differential diagnosis. Interprets clinical significance of diagnostic study results.

Patient care

  • Clinical reasoning. Organizes clinical facts (e.g., history, exam, tests, consultations, etc.) to compare and contrast diagnoses being considered, resulting in a prioritized differential diagnosis.
  • Organize and prioritize patient care. Organizes and prioritizes the simultaneous care of multiple patients with efficiency.
  • Patient management. Develops an interdisciplinary management plan for common and typical diagnoses.

Interpersonal and Communication

  • Patient and family centered communication. Establishes a culturally competent and therapeutic relationship with patients/families. Communicates with sensitivity and compassion. Elicits patient/family values and incorporates awareness of these in communications with patient/family and in patient care. Acknowledges uncertainty and conflict and escalates to more experienced APPs, etc., as appropriate.
  • Interpersonal and team communication . Formulates specific questions for consultation with supervising physician and tailors communication strategies as appropriate to promote collaborative practice. Uses bi-directional communication within the inter-professional team.
  • Communication within Health Care System. Concisely documents updated, prioritized, diagnostic and therapeutic reasoning in the patient record. Aligns type of communication with message to be delivered (e.g., direct and indirect) based on urgency and complexity

Practice–based learning and improvement

  • Evidence-based and informed practice. Critically analyzes data and evidence (from literature), integrated with patient preference, and applies to the care of patients.
  • Reflective practice and commitment to personal growth. Proactively seeks feedback. Designs and implements a learning plan, incorporating feedback and performance data.

Education And/or Experience Required

  • Education:
    • Physician Assistant: Graduation from a physician assistant program accredited, at the time of graduation, by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA).
  • Experience: Minimum experience (minimum years & type of experience): At least 2 years of APP experience.
License And/or Certification Required

  • Physician Assistant: Certified by the National Commission on Certification of Physician Assistants, Inc. (NCCPA). Licensed by the Connecticut Department of Public Health.
