Overview
We are seeking a dedicated and compassionate RN Memory Care Coordinator to lead and support our specialized memory care programs. Duties
- Develop and implement individualized care plans tailored to residents’ cognitive and physical needs, ensuring adherence to best practices in memory care.
- Conduct thorough patient assessments, including vital signs, physiological evaluations, and behavioral observations to inform treatment strategies.
- Coordinate discharge planning and post-acute care services to facilitate seamless transitions between inpatient, outpatient, and community settings.
- Oversee documentation of medical records, ensuring compliance with HIPAA regulations and maintaining accurate case management records.
- Collaborate with multidisciplinary teams—including physicians, therapists, social workers, and family members—to optimize patient outcomes.
- Monitor infection control protocols and ensure safety standards are maintained within the memory care unit.
- Provide education and support to staff regarding geriatrics, behavioral health management, and specialized nursing practices for patients with cognitive impairments.
Qualifications
- Valid RN licensure with current state certification; prior experience in geriatrics or behavioral health preferred.
- Critical care experience including ICU or emergency room settings is highly desirable; Level I Trauma Center or Level II Trauma Center experience is a plus.
- Demonstrated knowledge of medical terminology, physiology, and acute care procedures relevant to complex patient populations.
- Experience with hospital medicine, post-acute care, or outpatient clinic environments that involve patient assessment and discharge planning.
- Strong understanding of HIPAA regulations, medical records management, and utilization management principles.
- Proven ability to coordinate case management activities effectively within hospital or healthcare settings.
- Excellent communication skills with the ability to educate staff on medical-surgical practices, diagnostic evaluation, and patient service excellence.
