Employee will be responsible for the follow up and coordination of the patient referral, labs, and diagnostic imaging process for specialty care as determined by the clinic's medical providers and maintaining an established system of patient tracking and reporting. Excellent communication and organizational skills required. Duties will include but are not limited to: carrying out the entire referral process, referral tracking, patient phone calls, follow up to orders for testing, etc. Will be required to staff the floor as needed.
- Reviews, tracks, and follows up on Lab/DI/Referral tracking to meet patient satisfaction
- Reviews, tracks and follows up on Lab/DI/Referrals per policies and procedures and must be able to meet the mandatory guidelines and requirements
- Maintains ongoing documentation of issues encountered while performing QA tracking duties
- Identify issues and inform referral sources of any updates
- Be an advocate and resource to the patients as you are tracking referrals
- Maintain an ongoing communication with community partners to improve in the Referral/Lab/DI tracking documentation.
- Determines urgency of cases as tracking them and reporting to the appropriate person.
- Communicate with nurses, providers as needed.
- Function as call nurse as needed
- Education: High School diploma or equivalent. Medical Assistant diploma from a vocationalinstitution, or a community college course in medical assisting required.
Experience: Minimum two years direct patient care experience required.
