Weight Team/CNA Role: 

Provides direct and indirect resident care activities under the direction of a Registered Nurse or Licensed Practical Nurse or Registered Dietitian. Assists residents with daily living, provides personal care, comforts and assists in the maintenance of a safe and clean environment for residents. Provides accurate weights of residents daily as scheduled and as needed. Supports the mission of the The Health Center at Standifer Place. Exemplifies The Better WAY through leadership and demeanor. Represents the center in a positive manner both in the medical community and in the community as a whole. 

Weight Team/CNA Qualifications:

1. Must have ability to meet performance requirements

2. Must attend in-service programs, as assigned, to learn procedures, develop skills, and to meet state and federal requirements for health care providers and to maintain certification.

3. Personal Qualifications:

1. Sensitive to our residents' physical and psycho-social needs

2. Ability to follow oral and written instructions.

3. Able to read and write so that center charting requirements can be met.

4. Pleasant and cheerful personality.

5. Tactful and courteous approach with residents and visitors.

6. Treat all resident information as confidential material by complying with all HIPPA regulations.

7.     Adhere to dress code as directed by center policy.

Weight Team/CNA Performance Requirements: 
As a Weight Team/CNA you are responsible for handling and caring for residents in a manner conducive to their safety and comfort licensed nurses and residents' physicians. 

1. Physical Demands:
1. Able to be on feet  up to 12 hours per day.
2. Able to lift 60-70 lbs. on a frequent basis.
3. Able to bend, stoop, squat or twist frequently in a day.
4. Able to see and hear adequately in order to respond to auditory and visual requests from residents.
5. Able to speak clearly to adequately communicate with residents.
6. Upper extremity mobility which does not impede fine motor coordination and manual dexterity.
7. Report any medical conditions, such as communicable disease that may interfere with providing direct resident care.

2. Specific duties and Responsibilities Required:

    1. Understands and practices universal precautions with all residents.

    2.   Performs all assigned tasks/procedures and reports to charge nurse if tasks not completed.

    3. Assists in the preparation for admission of residents.

    4. Assists in and accompanies residents during admission, transfer and discharge procedures.

    5. Provides postmortem care.

    6. Provides morning care, with may include bed bath, oral hygiene, combing hair, back care, dressing residents, changing bed linen, cleaning over-bed table and bedside stand, straightening room, and other general care as necessary throughout the day.

    7. Provides evening care, which includes hands/face washing as needed, oral hygiene, special skin care, freshening linen, cleaning over-bed tables, straightening room, and other general care as needed.

    8. Provides general nursing care such a positioning resident: lifting and turning residents, applying/utilizing special equipment: assisting in use of bedpan, urinal or commode, and ambulating

    9. Takes and records temperature, pulse, respirations, weight, blood pressure and intake and output measurements.

    10. Adheres to policies and procedures of the Department Of Nursing 

    11.Participates in socialization activities on the unit and assists resident to activities. 

    12. Turns and positions as ordered and/or as needed, making sure no tough surfaces are in direct contact with the body. Lifts and turn with proper and safe mechanics and with available resources.

    13. Checks for reddened areas or skin breakdown and reports to RN or LPN.

    14. Ensures that residents are dressed properly and assists as necessary. Ensures that clothing is properly stored in dressers or on hangers. Ensures that all residents are clean and dry at all times. 

    15. Checks unit for adequate linens and supplies.

    16. Restrains residents properly when ordered. Understands reason for restraint and suggests less restrictive measures if possible.

    17. Accompanies residents to appointments as directed. 

    18. Provides reality orientation in daily care.

    19. Prepares residents for medical staff ordered diet, removes food trays and assists with meals or feeds resident. 

    20. Distributes drinking water and other nourishment to residents.

    21. Performs general care activities for residents in isolation, adhering to the center's infection control policies.

    22. Answers residents' call lights, anticipates residents' needs and makes rounds. 

    23. Assists residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aides and prosthetic devices).

    24. Collect and transports specimens as directed, following infection control P and P. 

    25. Reports (and records when appropriate) changes observed in condition or behavior of residents. 

    26. Knows the Nursing Care Plans.

    27. Assists in maintaining a safe and clean environment, reports equipment or weighing equipment that is out of order. Keeping weighing equipment charged. Report other problems to the charge nurse, maintains appropriate handling of clean and soiled linen and stores supplies on the unit. 

    28. Establishes and maintains interpersonal relationship with residents, family members and other center personnel, while assuring confidentiality of resident information.

    29. Practices careful, efficient and non-wasteful use of supplies and linen and follows established charge procedures for resident charge items.

    Other Demands: 

    1. Willingness to work with the realization that errors may have serious consequences for residents. Recording an accurate body weight is an important part of any nutritional screening tool as well as other interventions that may arise as part of the residents treatment such as Fluid loss/gain and medication adjustments.

    2. Patience and tact in dealing with residents.

    3. Willingness to perform a variety of simple repetitive tasks.

    4. Subject to night, weekend and holiday duty.
