Key Responsibilities
- Deliver behavioral health assessments, individual and group therapy, and intensive case management.
- Conduct home and community visits to ensure client progress and well-being.
- Collaborate with interdisciplinary teams and external partners to coordinate care.
- Support clients with budgeting and financial literacy as part of representative payee services.
- Maintain thorough and timely documentation in the electronic health record.
- Master’s degree in Social Work (LMSW or LCSW-C required).
- At least 1 year of experience in therapy and case management for individuals with substance use and mental health disorders (preferred).
- Knowledge of Harm Reduction, Motivational Interviewing, and Housing First practices.
- Strong communication, organizational, and crisis intervention skills.
- Valid Maryland driver’s license and access to a personal vehicle.
