Title:                                       Licensed Behavioral Health Provider (LMHC/LCSW/LMFT) 

Reports to:                             Behavioral Health Manager                             

Status:                                    Non-Exempt

Work Schedule:                     Full Time: Monday through Friday. Days/times may vary

 

Position Summary: 

 

Responsible for assessment and treatment of youth (primarily age 11-17) with mental, emotional, social or substance abuse problems, including abuse of alcohol, tobacco, and/or other related drugs. Activities may include individual, group therapy, crisis intervention, case management, client advocacy, prevention, and education. Services may be provided in varied settings at varied times, including in-office, school-based, clinical exam rooms, telehealth, off-site collaborative sites, permanent supportive housing (PSH) homes, homeless shelters, during homeless street outreach (encampments, streets, shelters, etc.). Ensures compliance with current federal and state standards, guidelines and regulations. Position will participate in Quality Improvement activities as needed.

 

 

Responsibilities & Expectations:

 

  • Provide mental health screenings and initial assessments to medically‐referred, school-referred, and community based youth aged 11-17 and their parents/guardians.  
  • Provide clinical therapeutic services to youth and adults, including individual and group therapy and coordination of treatment for participants with co-occurring disorders.
  • Provide short‐term, structured, evidence‐based psychotherapy as part of an appropriate treatment plan, both face-to-face and via telehealth.
  • Utilize various evidenced-based, clinical assessment tools (including PHQ-9 and 9A, CFARS, GAD, MDQ, DAST, AUDIT, etc.) to determine prioiritize needs, determine appriate level ov care, and track symptom improvement. 
  • Coordinate and collaborate with primary care providers, parents, caregivers, teachers, and other professionals to ensure coordinated care and improve health outcomes.
  • Facilitate referrals to other services (e.g. substance abuse treatment, specialty care and community resources as needed.
  • Work closely with Psychiatric Nurse Practioner for case consultation and medication management.
  • Provide crisis intervention as needed, including to clients experiencing acute distress, suicidal thoughts, self-harming behaviors,etc. 
  • Maintain accurate and timely records of activities and services provided to each client.
  • Assist with program development and recruitment of client population through marketing in schools, training OCPS/OBFH staff re: referrals, scheduling, etc. 
  • Administer, collect, track, and report pre/post clinical surveys to track outcomes. 
  • Assist with written quarterly summaries of program outcomes. 
  • Participate in and/or facilitate multi-disciplinary and multi-agency team meetings.
  • Work to meet patient needs effectively and resolve individual barriers through follow‐up, advocacy, and collaboration with OBFH staff and other community service providers
  • Other duties as assigned to support the mission of the organization.

 

Requirements:

 

  • Masters Degree from accredited program in Social Work or related field. 
  • Active license in the State of Florida (LCSW, LMFT, LMHC) 
  • Two years experience in a school-based setting providing behavioral health services to youth age 11 and above.
  • Experience working with varied populations, including youth in school-based and community settings, the homeless population, and those with substance use, and mental disorders. 
  • Experience with electronic health record documentation. 
  • Must have strong interpersonal, written and oral communication skills. 
  • Experience utilizing Microsoft Office suite. 
  • Knowledge or Integrated Behavioral Health in Community Health (preferred)
  • Knowledge of Federal, State and County regulations for FQHCs (preferred)
  • Clean driving record, ability to drive company vehicle(s) as needed.
