Key responsibilities include managing caseloads with routine visits, evaluations, and care starts, developing personalized care plans based on assessments and physician orders, and educating patients on conditions such as CHF, COPD, Diabetes, and Wound Care.

Qualifications include Missouri or Compact RN license, valid driver’s license, vehicle with insurance, and preferably experience in Home Health or Critical Care.
