We are seeking an experienced, autonomous Physician to provide primary and episodic care in skilled nursing facilities.
Key responsibilities include managing a personal patient panel, conducting annual wellness visits, addressing acute and chronic conditions, reviewing medications, and collaborating with specialists and staff to ensure seamless care. The role involves seeing approximately 16–25 patients daily, maintaining long-term relationships with residents and families, and participating in care conferences.
Qualifications include MD or DO from an accredited school, Board Certification in Internal Medicine, Family Medicine, or Geriatrics, and a valid, unrestricted license.
Skills and experience required are strong clinical judgment, ability to work independently in a fast-paced environment, and preferably, experience in skilled nursing or long-term care.
