Role Overview: Provide direct nursing care in patients' homes, following physician orders, and coordinate care activities to ensure high-quality patient care within the assigned geographic area. Work under supervision of a Registered Nurse or designated supervisor.

Key Responsibilities
- Deliver clinical services within the scope of practice defined by state laws, adhering to the plan of care and infection control procedures
- Document patient visits thoroughly in electronic medical records and visit verification systems
- Participate in care coordination by reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, and healthcare team members
- Current, unrestricted LPN licensure and CPR certification
- Valid driver's license, vehicle insurance, or access to transportation
- At least one year of LPN experience
- Ability to handle diverse home environments and provide care regardless of client background
- Home care experience
- Flexibility, independence, strong communication, organizational skills, and multitasking ability
