Role Overview: Provide dedicated, one-on-one pediatric home health nursing care, collaborating closely with RNs and clinical leadership to ensure consistent and confident patient support. Focus on building meaningful relationships and delivering care tailored to each child's needs in a safe, home environment.

Role Overview: Provide dedicated, one-on-one pediatric home health nursing care, collaborating closely with RNs and clinical leadership to ensure consistent and confident patient support. Focus on building meaningful relationships and delivering care tailored to each child's needs in a safe, home environment.
Responsibilities Include
- Administering skilled nursing care to pediatric patients
- Following clear care plans with RN oversight
- Supporting patients with G-tube, trach, vent needs or training for these if necessary
- Ensuring safety protocols and CDC guidelines are followed
- Utilizing the Employee Portal for shift management and communication
- Valid Pennsylvania LPN or Multistate License
- Current TB test and BLS CPR certification (in person)
- Valid driver's license
- Experience or willingness to train in G-tube, trach, vent care
