Responsibilities

Administer medications and monitor patient conditions
Support and implement individualized care plans
Accurately document care activities in EHR systems

Qualifications & Skills

Valid LPN license
Strong clinical skills and attention to detail
Ability to work independently and adapt to flexible schedules
Excellent communication and documentation abilities

Key Duties: Deliver compassionate patient care, support care planning, ensure accurate documentation, and participate in a clinician-first, technology-enabled platform that promotes growth and transparency.
