What are the responsibilities and job description for the Field Nurse Practitioner (Full-Time) position at Health Care Administrators Association® (HCAA)?
Provides primary medical care and care coordination to diverse patient populations in non-clinical settings, including in-home, community, and nursing facilities. Performs comprehensive evaluations, addressing both chronic and acute complaints, and develops personalized care plans. Orders and interprets diagnostic tests, seeks specialist consultations when needed, and prescribes medications and procedures within scope. Engages in community outreach through pop-up clinics and manages post-discharge follow-up to reduce hospital readmissions. Requires driving within assigned regions, maintaining appropriate licensure, and collaborating with a healthcare team. Must have at least 1 year of nurse practitioner experience, active FNP license, DEA certification, BLS, and strong communication and problem-solving skills. Preferred experience includes home health, underserved populations, immunizations, and bilingual abilities.
