Responsibilities
- Collaborate with clinic teams and participate in provider huddles to share patient data and improve outcomes.
- Coordinate preventive care, facilitate patient transitions, and resolve issues to ensure timely follow-up.
- Manage patient information in electronic records, schedule appointments, and connect patients to community resources.
- Support care interventions aimed at cost savings and health improvements.
- Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) or recognized Medical Assistant certification.
- Proficiency in Microsoft Office, medical terminology, and excellent communication skills.
- Strong organizational, problem-solving, and compassionate patient navigation abilities.
