Position Summary
We are seeking a dedicated and licensed Social Worker (LSW) to join our residential group home team. The Social Worker will play a critical role in service planning, advocacy, staff training, and therapeutic support for youth in care, in compliance with DCY Group Home regulations and the QRTP (Qualified Residential Treatment Program) model. This position requires strong clinical skills, organizational ability, and a commitment to trauma-informed, youth-centered care.
Minimum Qualifications
- Active Licensed Social Worker (LSW) (required)
- Knowledge of DCY Group Home Codes, CARF standards, and QRTP requirements a plus.
- Experience working with adolescent youth in residential or child welfare settings strongly preferred
- Strong documentation, communication, and training skills
Key Responsibilities
- Complete, update, and maintain 30-day, 60-day, and 90-day Individual Service Plans (ISPs) in accordance with regulatory timelines
- Assist with intake documentation, including initial assessments and service planning materials
- Conduct and update DAF (Diagnostic Assessment Form) assessments as required
- Provide individual counseling and support to youth, utilizing trauma-informed and strengths-based practices
- Serve as a primary advocate for boys in care, collaborating with treatment teams, schools, caseworkers, and external providers
- Train and coach direct care staff monthly on the SITCAP QRTP model, ensuring fidelity to program expectations
- Monitor implementation of treatment goals and support behavioral interventions
- Ensure compliance with DCY regulations, agency policies, and documentation standards
- Participate in multidisciplinary team meetings, case reviews, and audits
- Perform other duties as required to support program integrity and youth outcomes
