Overview
We are seeking a highly skilled and experienced RN Supervisor to lead our Home Care team and soon to be Home Health and Hospice. This leadership role involves overseeing patient care, managing nursing staff, ensuring compliance with healthcare standards, and coordinating complex home health services. The ideal candidate will possess extensive clinical expertise across various settings, including acute care, geriatrics, behavioral health, and specialized areas such as dialysis, IV infusion, and dementia care. Strong management skills and proficiency with electronic health records are essential to ensure high-quality patient outcomes and operational efficiency. This role starts as PER DIEM but will grow into FULL TIME.

Duties

  • Supervise and coordinate nursing staff providing home health services, ensuring adherence to care plans and clinical protocols.
  • Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations.
  • Oversee medication administration, IV insertion, catheter care, tube feeding, and other complex clinical procedures following sterile techniques and infection control standards.
  • Manage documentation using EMR/EHR systems such as Epic, Cerner, Athenahealth, and eClinicalWorks to ensure accurate medical records in compliance with HIPAA regulations.
  • Develop and implement discharge planning, case management strategies, and utilization review processes aligned with NCQA standards.
  • Provide clinical guidance on specialized care areas including dementia care, behavioral health, pediatric (including neonatal ventilator), neonatal care, and adult long-term care.
  • Ensure compliance with healthcare laws such as Workers' Compensation Law and maintain standards for infection control and sterile processing.
  • Lead quality improvement initiatives related to patient safety, pain management, acute pain protocols, and diagnostic evaluations.
  • Collaborate with multidisciplinary teams across hospital medicine, emergency medicine, ICU/CCU units (including Level I & II Trauma Centers), and outpatient clinics to coordinate seamless patient transitions.
  • Supervise training programs for staff on ICD-9/ICD-10 coding, CPT coding, medical terminology, anatomy/physiology knowledge, and advanced clinical procedures like cardiac catheterization or spinal tap.
  • Monitor staff performance through evaluations focused on nurse management skills and team management practices.

Skills

  • Extensive clinical experience in diverse settings including ICU experience, emergency medicine, medical-surgical units, primary care, geriatrics, pediatrics (including Toddler & Neonatal Care), behavioral health, and home care environments.
  • Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with Athenahealth or eClinicalWorks preferred.
  • Strong knowledge of medical documentation standards adhering to HIPAA regulations; experience with medical imaging and laboratory procedures is advantageous.
  • Skilled in patient assessment techniques including vital signs measurement, diagnostic evaluation interpretation, venipuncture/phasbotomy/injections administration (including IV infusion), airway management (ventilators), suturing techniques, gastrostomy tube care (Gastric lavage), Hoyer lift operation for mobility assistance.
  • Ability to manage complex cases involving dementia or Alzheimer’s disease; experience working with people with disabilities or developmental disabilities is highly valued.
  • Leadership capabilities in supervising nursing teams; experience managing staff schedules and conducting performance reviews.
  • Knowledge of healthcare coding systems such as ICD-9/ICD-10/CPT codes; familiarity with DRGs for billing purposes is beneficial.
  • Competence in infection control practices; understanding of sterile processing procedures; experience in laboratory specimen collection/processing is a plus.
  • Excellent communication skills for triage situations; ability to provide health coaching and discharge planning effectively. Pay: $29.00 - $40.00 per hour

Expected hours: 2.0 – 10.0 per week
