A Mental Health Case Manager plays a vital role in the therapeutic and recovery process of individuals dealing with mental health issues. Mental Health Case Manager Responsibilities & Duties

*Conduct comprehensive assessments of clients' mental health needs.

*Develop and implement individualized case management plans.

 *Coordinate with healthcare providers, social services, and community resources.             

 *Provide crisis intervention and emotional support.                                              

 *Advocate for clients' needs and rights.               

 *Monitor and evaluate client progress towards goals.

 *Maintain detailed and accurate case records.

 *Facilitate access to mental health services and resources.

 *Educate clients and families about mental health conditions and treatments.

 *Assist clients with managing medications and appointments.

 *Participate in multidisciplinary team meetings.

 *Conduct follow-up visits to ensure continuity of care.

 *Offer supportive counseling and problem-solving strategies.                                             

 *Develop and facilitate support groups.         

  *Collaborate with legal and protective services when necessary.                     

Mental Health Case Manager Qualifications & Skills

*Bachelor’s degree in social work, psychology, or a related field.

 *1-2 years of experience in mental health case management.

*Experience working with diverse populations.

*Advanced knowledge of mental health disorders and treatments.

*Strong crisis intervention skills.

*Proficiency in electronic health record (EHR) systems.

*1-2 years of experience in mental health or social services.

*Strong organizational and time-management skills.

*Excellent communication and interpersonal skills.                

*Ability to work independently and as part of a team.

*Valid driver's license and reliable transportation.

*Knowledge of local community resources and services.                              

*Understanding of confidentiality and HIPAA regulations
