We are seeking an experienced Hospice LVN Case Manager who understands the unique responsibilites of hospice case management and can effectively coordinate patient care within an interdisciplinatary team.
Position Summary
The Hospice LVN Case Manager is responsible for managing a caseload of hospice patients and coordinating all aspect of their care. The LVN Case Manager works closely with physicians and the interdisciplinary team to ensure patients receive compassionate, appropriate, and compliant hospice care.
Key Responsibilities
- Manage and coordinate a caseload of hospice patients.
- Perform routine and PRN patient assessments.
- Implement and update individualized care plans.
- Provide skilled nursing care and symptom management.
- Educate patients and families on disease progression and end-of-life care.
- Coordinate care with physicians, RNs, social workers, chaplains, and home health aides.
- Document all patient visits accurately and timely in the hospice EMR system.
- Monitore patient condition and report changes to the clinical team.
- Ensure compliance with Medicare hospice regulations and agency policies.
- Familiarity with discharge planning processes, utilization management strategies, and managed care principles improves service delivery efficiency.
- Strong communication skills to effectively collaborate with multidisciplinary teams across inpatient and outpatient settings.
- Ability to navigate HIPAA regulations diligently while maintaining confidentiality of sensitive patient information.
Required Qualifications
- Active California LVN Licence
- Minimum 2-3 years Hospice Case Manager experience (REQUIRED)
- Experiance managing hospice patient caseload
- Strong Knowledge of Medicare hospice guidelines
- Excelent patient assessment and clinical documentation skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation
Prefered Qualifications
- Experiance with HospiceMD or other hospice EMR systems
- Experience participating in IDT meetings
- Bilingual (English/Spanish) preferred
