Responsibilities
- Observe and monitor patients’ conditions regularly, documenting changes accurately in electronic health record (EHR) systems while maintaining strict patient confidentiality in accordance with HIPAA regulations
- Assist clients with activities of daily living such as bathing, dressing, grooming, toileting, and mobility using proper patient transfer methods and equipment like Hoyer lifts or gait belts
- Administer medications safely following medical terminology guidelines and home healthcare regulations under supervision or as authorized
- Perform vital signs assessments including blood pressure, pulse, temperature, and respiration rates to track health status effectively
- Support patients with specialized needs such as Alzheimer’s care, dementia care, memory care, and end-of-life support with patience and compassion
- Maintain a clean and safe environment by assisting with laundry, cleaning tasks, meal preparation, and ensuring proper infection control practices are followed
- Respond promptly to patient calls or emergencies using first aid skills and basic life support (BLS) techniques when necessary
Qualifications
- Certified Nursing Assistant (CNA) license or Personal Care Assistant (PCA) certification required; relevant experience preferred
- Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly desirable
- Knowledge of medical terminology and medical documentation procedures essential for accurate record keeping
- Familiarity with EMR/EHR systems for charting patient information efficiently
- Experience working with individuals with disabilities or developmental challenges is a plus
- Ability to perform heavy lifting safely and use patient transfer equipment effectively
- Valid CPR and first aid certifications; ongoing training encouraged to stay current with best practices
- Strong communication skills for engaging with patients, families, and healthcare team members effectively
