Registered Nurse / Case Manager – Hospice

Position Title: Registered Nurse / Case Manager
Immediate Supervisor: Director of Clinical Services
Employment Type: Full-Time (or as specified)
Location: Butte, Montana

Position Summary
The Registered Nurse / Case Manager is responsible for planning, coordinating, and providing professional hospice nursing care to patients with life-limiting illnesses and their families. This role follows the nursing process to deliver holistic, compassionate care in accordance with the patient's individualized plan of care. The RN collaborates with the interdisciplinary group (IDG), physicians, and other team members to ensure high-quality, patient-centered services that promote comfort, dignity, and quality of life.

Key Responsibilities

  • Conduct initial and ongoing comprehensive assessments of the patient and family to determine hospice needs, including medical history, physical examination, vital signs, mental status, nutritional status, and home environment evaluation.
  • Develop, implement, and maintain the individualized plan of care in collaboration with the IDG, attending physician, and Hospice Medical Director.
  • Coordinate patient care with other IDG members, contracted services, and external agencies to ensure continuity and appropriateness of services.
  • Evaluate the effectiveness of hospice services on an ongoing basis and revise the plan of care as needed.
  • Perform skilled nursing procedures, treatments, and symptom management (e.g., pain control, medication administration, wound care) as ordered by the physician.
  • Observe and report changes in the patient's physical, emotional, or environmental condition to the physician and team; recommend order changes or adjustments to the plan of care.
  • Provide education and counseling to patients and families on the dying process, symptom management, dietary needs, healthcare decisions, and end-of-life care.
  • Supervise and evaluate Hospice Aides (minimum every 14 days) and LPNs (minimum every 30 days, or per state regulations); provide guidance and document supervisory visits.
  • Submit accurate, timely clinical notes, visit schedules, and other documentation.
  • Participate in IDG meetings, case conferences, QAPI activities, staff development, and on-call duties (including holidays) as required.
  • Perform death visits, bereavement support, and discharge planning as per agency policy.
  • Maintain current knowledge of pharmacology, federal/state regulations (including Medicare/Medicaid), and best practices in hospice care.
  • May provide coverage for other nurses or assist with informational/evaluation visits as needed.

Job Conditions

  • Work may be stressful and emotionally demanding.
  • Requires access to patients' homes (which may not be wheelchair accessible).
  • Physical demands include walking, standing, stooping, lifting/turning patients (minimal to maximum), and sufficient hearing, vision, and dexterity for assessments and care delivery.

Has access to confidential patient medical records; must adhere to HIPAA and agency confidentiality policies.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Current, unrestricted Registered Nurse license in the State of Montana.
  • Minimum of one (1) year RN experience; hospice experience preferred.
  • Strong assessment, problem-solving, organizational, and time management skills; knowledge of case management processes.
  • Excellent verbal and written communication skills.
  • Valid driver's license, reliable insured automobile in good working order, and ability to travel to patients' homes.
  • Ability to administer pharmaceuticals, perform medication reconciliation, and conduct nursing procedures within Montana scope of practice.
  • All competencies verified prior to independent patient care (especially for infrequently performed procedures).
  • Compassionate, empathetic, and comfortable working with patients facing end-of-life issues.

Job Type: Full-time

Pay: $35.00 - $45.00 per hour
