Title: Sustainability Coach

Immediate Supervisor: Clinical Operations Manager

Position Summary
The Sustainability Coach (Health Educator) supports the continuous quality improvement cycle using HD Nursing’s Prevention Programs. The person in this role works with all members of a hospital clinical leadership team to provide a consistent, highly satisfied customer experience. Daily work is focused on planning, communicating, and supporting change from the client’s current state to effective and robust use of the HD Nursing’s Prevention Programs.

Standard Duty Areas
Project Coordination

• Oversee the post-go-live phase of HD Nursing’s Prevention Program implementations

• Communicate milestones, growth, gaps, and next steps to clients

• Coordinate project schedules, resources, and information for implementations.

• Maintain accurate and thorough records of client interactions.

• Follow standard processes and policies for implementation.

• Communicate changes or variances to expected progress as appropriate.

• Drive quality metrics for clients using established HD Nursing program tools.

Client Support/Customer Service

• Provide cheerful, efficient customer solutions to client stakeholders.

• Proactive identification and responsiveness to client needs.

• Ensure client needs are met as projects evolve.

• Act as a conduit for client feedback to the HD Nursing leadership team.

• Demonstrates professional polish and skill at building client relationships quickly and sustaining them credibly.

Professional Development

• Maintain clinical proficiency as an expert on HD Nursing’s Prevention Programs.

• Demonstrate leadership qualities of poise, thoughtfulness, and clarity while interacting with clients.

Required Skills

• Demonstrates a warm, professional presence leading meetings and connecting with others in a virtual setting

• Minimum clinical background: Certified Health Educator (CHES or MCHES) with 3-5 years of experience.

• Unencumbered clinical license in a US State if applicable

• Familiar with electronic health record documentation

• Proficient in Microsoft Word, PowerPoint, Outlook Email

• Basic knowledge of Adobe PDF and Excel

Position Directly Supervises: None

Decision Making: Use independent judgment when solving problems and helping clients during all phases of implementation. Be able to answer questions both clinical and programmatic in nature related to HD Nursing’s Prevention Programs.

Budget Responsibility: None

Interaction: HD Nursing clients, industry professionals and internal HD Nursing Staff.

Accountability: Accountable to the successful implementation of the HD Nursing Program for assigned clients as evidenced by internal compliance to project management standards, client performance and client retention.
