What are the responsibilities and job description for the Manager, Continuity of Care (Case Management) position at HCTHealthcare?
HCT seeks an experienced nursing professional to serve as the Manager of Continuity of Care for a 270 bed hospital in California. Responsibilities include, but are not limited to:
- Provide hands-on leadership for a hospital case management department.
- Ensure case management policies and processes are consistently followed, educate staff on effective case management practices, improve multidisciplinary rounding functions, strengthen staff accountability, and support departmental operational excellence.
- Oversight to include discharge planning, utilization management, length of stay management, avoidable-day and discharge-barrier reduction.
- Establish outstanding rapport and coordination with team, staff, and physicians.
Requirements:
- Education: BSN degree required.
- Experience: Minimum 5 years progressive case management leadership experience in an acute care hospital is required.
- Licensure: Current CA RN license required.
- Certifications: BLS through AHA required or completed during onboarding.
- Computer Skills: Epic
