Role and Responsibilities 


Responsible for developing and maintaining client relations and intake of direct referrals made to HCR. 

 

Essential Functions 

  • Cultivate relationships with our physicians and hospitals to facilitate the best Home Care solutions for their patients. 
  • Monitor HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB). 
  • Monitor Intake Team email. 
  • Produce timely, accurate data entry. 
  • Responsible for entering orthopedic prescreens into HCHB according to agency protocol. 
  • Provide phone support to the department and triage of calls to Intake staff.  
  • Provide excellent customer service to internal and external customers. 
  • Participate in department and agency meetings. 
  • Other duties as assigned.

Education Requirements 

  • LPN licensure

 

Qualifications and Requirements 

  • Minimum of 4 years' experience in a clinical setting. 
  • Exceptional customer services and communication skills required. Must be highly proficient and efficient working with a computer and be detail oriented and able to work in a fast-paced environment.  

 

Work Environment 

The Client Relationship Specialist is primarily in an office setting and may be exposed to outdoor conditions. 


The working conditions are classified as sedentary work: 

  • Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
