RN - Care Manager (Skilled Nursing Facility)

Compensation: $50.00 – $61.00 an hour

Our client is seeking a Skilled Nursing Facility (SNF) Care Manager to lead and oversee assigned care management staff. This individual is responsible for optimizing member care coordination within contracted SNFs, partnering with departmental leadership, and collaborating across the organization to ensure seamless continuity of care, high-quality outcomes, and efficient length of stay (LOS) management.

Responsibilities:
  • Provide direct oversight, resources, and clinical guidance to assigned SNF care management staff. Monitor employee performance, deliver coaching/training, and complete timely professional evaluations.
  • Track and analyze key SNF performance metrics—including Length of Stay (targeting under 13 days), Emergency Department utilization, and readmission rates. Leverage this data to develop operational strategies and present findings to executive management.
  • Ensure staff regularly evaluate patient care plans to promote steady rehabilitation progression. Audit specialty referrals within internal tracking systems to troubleshoot and eliminate any delays in care.
  • Participate in daily hospital reports to review acute-to-SNF transfers. Act directly on clinically complex or intensive psychosocial cases to facilitate timely transitions to hospice or alternative long-term placements.
  • Build strong professional relationships with contracted SNF personnel and providers. Ensure facility adherence to clinical and nursing service contracts while serving as the primary escalation point for care disputes.
  • Act as an organizational ambassador by practicing active listening, keeping multidisciplinary teams informed of workflows, and ensuring clear communication channels for both daily activities and unexpected clinical escalations.
Requirements:
  • Current, active California Registered Nurse (RN) License. Graduate of an accredited nursing program; a Bachelor of Science in Nursing (BSN) is highly preferred.
  • Minimum of 3 years of recent acute care nursing experience. Proven experience utilizing clinical judgment and critical thinking to manage staff or lead clinical initiatives is highly valued.
  • Strong familiarity with Level of Care, intensity of services, severity of illness, discharge screening, and SNF/Home Health/Hospice admission criteria.
  • Proficiency with Microsoft Office applications (Word, Excel, Outlook, PowerPoint) and general comfort adapting to healthcare tracking software and database systems.
  • Excellent communication and multitasking abilities to manage strict deadlines. Valid California driver’s license with proof of auto insurance required.


Physical Demands & Work Environment:
  • Desk and field-balanced role involving sitting (40%), standing (25%), and walking (35%).
  • Requires frequent reaching above the head (20%) and occasional lifting of equipment/supplies up to 40 lbs.
  • Highly technical workspace utilizing computers, cell phones, and tablets for more than 6 hours per day
