Overview
We are seeking a compassionate and highly skilled Pediatric Home Care Nurse to provide specialized nursing care to a child in their home environment. This role requires a dedicated professional with experience in pediatric care, critical care, and familiarity with advanced medical procedures. The Pediatric Home Care Nurse will work closely with families, caregivers, and healthcare teams to deliver comprehensive, family-centered care that adheres to all regulatory standards and best practices.
Responsibilities
- Deliver direct nursing care to pediatric patients in their homes, including medication administration, tube feedings, and wound care such as suturing and ostomy management.
- Perform comprehensive patient assessments, monitor vital signs, and evaluate physiological responses to treatment plans.
- Monitor O2 sats.
- Coordinate with physicians and multidisciplinary teams to develop and implement individualized care plans tailored to each child's needs.
- Educate families and caregivers on disease management, medication administration, device usage, and safety procedures.
- Recognize urgent health issues through triage skills and escalate care appropriately during emergencies or acute episodes.
- Assist with discharge planning and transition of care from hospital or clinic settings to home environments.
- Maintain compliance with all healthcare regulations including Workers' Compensation Law and NCQA standards while ensuring safety standards are met.
Experience
- Proven experience in pediatric nursing is required; experience with neonatal ventilators, PICU or NICU settings is highly desirable.
- Background in critical care environments such as ICU, emergency medicine, trauma centers (Level I or II), or hospital medicine is preferred.
- Familiarity with complex procedures including catheterization (urinary or vascular), dialysis, cardiology procedures like cardiac catheterization, and advanced airway management techniques such as airway management for ventilator-dependent children.
- Experience working with children who have developmental disabilities, behavioral health needs, Alzheimer's or dementia care is advantageous.
- Proficiency in medical documentation using EMR/EHR systems like eClinicalWorks or Athenahealth is essential.
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, laboratory procedures including specimen collection/processing, and diagnostic evaluation enhances clinical effectiveness.
- Additional skills such as phlebotomy, administering injections, wound suturing, ostomy care, gastrostomy tube feeding management (G-tube), and infection control are valuable assets.
