Job Summary

The Assistant Clinical Supervisor (RN) supports the Clinical Supervisor in overseeing day‑to‑day clinical operations, ensuring high‑quality home health care, regulatory compliance, and smooth coordination between field staff, clients, and administrative teams. This role combines direct patient care with leadership responsibilities, clinical oversight, staff support, and administrative functions essential to maintaining safe, effective, and compassionate home‑based services.

Key Responsibilities

Clinical Oversight & Quality Assurance

• Assist in supervising field clinicians (RNs, LPNs, HHAs/CNAs) to ensure adherence to care plans, agency protocols, and regulatory standards.
• Conduct supervisory visits, case conferences, and in‑home assessments as needed.
• Review clinical documentation for accuracy, completeness, and compliance with state and federal regulations.
• Support the development, implementation, and evaluation of individualized care plans.
• Participate in quality assurance activities, chart audits, and performance improvement initiatives.

Direct Patient Care

• Provide skilled nursing visits, assessments, and interventions when needed to support caseload coverage or urgent clinical needs.
• Monitor patient progress, update care plans, and communicate changes to the interdisciplinary team.
• Educate patients and families on disease management, medications, safety, and home‑care procedures.

Staff Support & Training

• Assist with onboarding, training, and mentoring new clinical staff.
• Provide ongoing coaching to ensure staff competency in clinical skills, documentation, infection control, and home‑care best practices.
• Support scheduling and coordination of field staff to ensure continuity of care.

Administrative & Operational Support

• Assist with intake coordination, case assignments, and communication with referral sources.
• Support compliance with agency policies, HIPAA, and state/federal home‑health regulations.
• Participate in interdisciplinary team meetings, care conferences, and agency planning activities.
• Help maintain accurate records, reports, and documentation required for audits and regulatory reviews.

Communication & Collaboration

• Serve as a liaison between field staff, office staff, patients, families, and providers.
• Communicate changes in patient condition promptly to the Clinical Supervisor and physician.
• Promote a culture of teamwork, professionalism, and patient‑centered care.

Qualifications

• Active Registered Nurse (RN) license in the state.
• Minimum 1–2 years of clinical nursing experience; home health experience strongly preferred.
• Strong understanding of home‑health regulations, documentation standards, and best practices.
• Excellent communication, leadership, and organizational skills.
• Ability to work independently, manage multiple priorities, and support staff in the field.
• Proficiency with electronic health records (EHR) and clinical documentation.

Work Schedule

• Full‑time position with a combination of office‑based administrative work and field‑based clinical supervision.
• Occasional flexibility required to support urgent patient needs or staff coverage.
