Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team in Gainesville. The ideal candidate will provide high-quality patient care in the comfort of patients' homes, ensuring their health and well-being through comprehensive assessments, care planning, and ongoing support. This role requires strong clinical skills and the ability to work independently while collaborating with a multidisciplinary team.
Responsibilities
- Conduct thorough assessments of patients' health status, including vital signs and diagnostic evaluations.
- Develop and implement individualized care plans based on patient needs and preferences.
- Provide direct patient care, including administering medications, wound care, and other necessary treatments.
- Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.
- Educate patients and their families on health management, discharge planning, and self-care techniques.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate comprehensive care.
- Maintain accurate documentation of patient interactions, care provided, and progress notes in compliance with regulatory standards.
- Participate in quality improvement initiatives to enhance patient care services.
Experience
- Current Registered Nurse (RN) license in good standing.
- Previous experience in home health, hospital medicine, ICU, clinic, or primary care settings is preferred.
- Proficiency in performing vital signs assessments and providing patient-centered care.
- Strong communication skills to effectively interact with patients, families, and healthcare team members.
- Ability to work independently while managing multiple priorities in a fast-paced environment.
