Job Summary And Qualifications
Position Summary: Under the general administration of the Swedish Stroke Medical Director, the Stroke Advanced Practice Nurse (APN) Program Coordinator is responsible for implementing and ensuring optimal inpatient care and outcomes for stroke patients (per the Joint Commission definition: TIA, hemorrhagic, and ischemic) according to evidence-based standards and guidelines through patient care, protocol development/compliance, data collection/analysis, quality improvement, disease prevention, research and professional education. The Stroke APN ensures multidisciplinary compliance with all state, national and Joint Commission Comprehensive Stroke Center requirements.
Position Requirements:
Licensure/Certification/Registration:
Current Colorado Registered Nurse License is required.
Colorado Clinical Nurse Specialist License or Nurse Practitioner License is preferred.
Current BLS and ACLS Certification through AHA or ARC.
Prescriptive Authority granted by the Colorado Board of Nursing and DEA Certificate is required if prescribing pharmacologic agents and/or writing medication orders.
Education:
Graduate of an Accredited School of Nursing and completion of an accredited program for advanced practice nurses Masters Degree required.Certification by National accrediting body in his/hers specialty area preferred.
Experience:
1-2 Years Advanced Practice Nurse (APN)provider experience preferred. 5-7 Years related previous experience as a nurse or advanced practice nurse preferred. Minimum field-of-experience, completion of an accredited program including preceptorship. Possesses advanced knowledge of theory and practice in professional nursing as evidenced by credentials, peer review and performance evaluation.
Knowledge/Skills/Abilities:
Working knowledge of healthcare data (from clinical to supply chain) with strong analytical skills
Excellent communication and data presentation skills
Clinical data and research strategy, with a focus on programmatic value improvement
Ability to manage multiple projects and timelines across different collaborative bodies
Instills confidence in collaborators with each interaction and exemplifies Swedish Medical Center’s standards of excellence
Self-driven with high internal standards and willing to set and strive for challenging goals
